Speaker Reimbursement - Due May 1
Fill out the linked Speaker Reimbursement Form and W-9 form. Email both forms, a letter from your bank or voided check as well as the corresponding receipts to us at educationalevents@signifyhealth.com by Thursday, May 1.
You may expense reasonable travel fees (flights and transportation to/from the airport OR mileage reimbursement) up to $500. Flights must be booked in Coach/Economy cabin. Receipts and/or verification from GSA for mileage need to also be attached to the reimbursement form. You are eligible for two and a half (2.5) days of Per Diem, maximum $165, with receipts or proof of purchase included.

- Review the recording from the Speaker welcome call, held on December 12
- Sign your speaker agreement from DocuSign
- Submit bio and high-resolution headshot for our website and promotional use through the Smartsheet form below. Please do not submit selfies as they cannot be properly cropped to fit our website theme.
- Register for the event as a speaker
- Book your discounted hotel room in the CVS room block
- Submit the first draft of your presentation at the Smartsheet form below
- Complete on-demand learning courses
- Book your dress rehearsal via Calendly
- Content finalized with the CVS Accountable Care Clinical Content Team, pending legal review.

- Virtual dress rehearsals take place
April 15, 2025 | (Breakout speakers only) Welcome & venue orientation
Join our event staff before the welcome reception to get a tour of the presentation space and any last minute updates prior to the Symposium kick-off.
April 15, 2025 | (General session speakers only) On-site tech rehearsal
General session speakers are required to attend a technical rehearsal before the welcome reception. This will include a walkthrough of the main stage, a lighting test, microphone tips, logistics and more.
