Population health pioneer awards

Born and raised in Stone Mountain, Georgia.

Graduated from Louisiana State University in 2007 with a Bachelors of Science in Kinesiology, and graduated from Louisiana College accelerated Bachelors of nursing in 2011. Nicole is married to Mikey and has three children, Tripp, Carlin, and Amelia.

She is a member of the Deridder Study Club, Louisiana Quality Network, State Health Improvement plan, Beauregard Prevention Coalition, Chair of Beauregard Community Outreach committee, Chair of the BHS SDOH committee and Chair of the Carter Club.

Her entire 13 year nursing career has been with Beauregard Health System, working in Cath Lab, Same day Surgery, GI lab, surgery, and recovery room. She began working as the Population Health Coordinator in 2020 and has lead the charge of primary care transformation into value based care. 

She is dedicated to improving the health and well-being of the community.

Mark Callery, D.O., is board-certified and has been practicing family medicine in the Tulsa area since 2006. He also serves as the Medical Director for Population Health at Utica Park Clinic.

Dr. Callery graduated from Oklahoma State University with a degree in biological sciences in 1999. He earned his osteopathic degree from the Oklahoma State University Center for Health Sciences in 2003. In 2004, he completed his internship at Tulsa Regional Medical Center. He completed his residency at Oklahoma State University Medical Center in 2006. He is a member of the American Osteopathic Association and the Oklahoma Osteopathic Association.  He is a trustee with the Oklahoma Osteopathic Association and chairs the bureau on physician wellness. He is also currently pursuing his Masters in Healthcare Administration.

Dr. Callery's personal interests include spending time with his wife and four children, playing golf and attending church.

Melissa Rumple, RN, CMCN, a nurse of 14 years, has honed her skills and knowledge to provide exceptional care to her patients. Employed at Hancock Health in Greenfield, IN for the past nine years, she is serving in her current role as Clinical Coordinator for Chronic Care Management.

Starting her journey at Ivy Tech in 2010, Melissa achieved her Associates of Science in Nursing while also leading as the Nursing Counsel President for her class, showcasing her leadership abilities and passion for the field. Her commitment to professional development led her to obtain her national certification as a Certified Managed Care Nurse in 2019, alongside becoming a certified CPR trainer, further enhancing her expertise and credentials.

Eager to expand her scope of practice, Melissa embarked on further education, enrolling at Walden University in 2020 to pursue a Master of Science in Nursing. She is focusing on becoming an Adult Gerontology Nurse Practitioner, demonstrating her commitment to advancing her skills and providing specialized care for the aging population.

In 2022 Melissa authored and was awarded $150,00.00 to improve cardiovascular health through the Indiana Department of Health, Health Issues and Challenges Grant. Melissa serves as the director, grant manager and project manager for the grant. The grant has been instrumental in developing a hypertension prevention and management program for Hancock County. Hancock Health’s Healthy Hearts has provided hypertensive screenings for over 200 individuals and continues to expand. Currently available in every Hancock Health family, internal medicine, and pediatric office, with expansion into the employee-based clinic by the end of 2024.

Beyond her career achievements, Melissa is a devoted wife and mother of two, balancing her responsibilities at work with her family life. Originally hailing from Mt. Summit, IN, she now resides in Decatur, IN.

Lawrence Eisenstein, MD, MPH, FACP, is Vice President and Chief Public and Community Health Officer for Catholic Health on Long Island. Prior to joining Catholic Health in August of 2022, 
Dr. Eisenstein served as Commissioner of Health for the Nassau County Department of Health, Mineola, New York for more than 11 years. Dr. Eisenstein also served as President of the New York State Association of County Health Officials (NYSACHO,) and he was on their Board of Directors for 9 years. 

Dr. Eisenstein is Board Certified in Infectious Diseases through the American Board of Internal Medicine. He completed an Internal Medicine Residency, Chief Residency, and Fellowship in Infectious Diseases at Winthrop University Hospital in Mineola, NY. Dr. Eisenstein began his Medical Education at Universidad Autonoma de Guadalajara before finishing it at New York Medical College, Valhalla, NY. He also earned his Master of Public Health degree from New York Medical College. 

Dr. Eisenstein has a long commitment to training the next generation in medicine and public health. He was Assistant Adjunct professor at Hofstra University’s Department of Health Professions and Kinesiology from 2011-2022. Dr. Eisenstein continues to serve as Clinical Assistant Professor, Department of Family, Population & Preventive Medicine, State University of New York at Stony Brook (since 2011.)

Dr. Eisenstein has presented numerous grand rounds, guest lectures, keynote speeches, and television,   newspaper, and other media interviews. He has received numerous honors, awards, and accolades including being named the Bellmore Herald’s Person of the Year in 2020, and leading the Nassau County Department of Health to the “Health Department of the Year Award” as given by the National Association of County and City Health Officials in 2018. Dr. Eisenstein was named to the New York State Ending the Epidemic Taskforce, and following Superstorm Sandy Dr. Eisenstein was able to serve his community by being appointed by the Governor as Community Rising Co-Chairman for the Bellmore/Merrick community. He continues to live in Bellmore, NY with his wife and daughters.
